What is the Arizona Living Well Institute?


The Arizona Living Well Institute (A.K.A. the Institute) is a public-private partnership encompassing all sectors of the healthcare community. The purpose of the Institute is to build statewide infrastructure and capacity for effective and efficient methods of promoting and implementing Evidence-Based Health Promotion Programs throughout the state.

Currently at the Institute


The Institute is currently partnered with the Arizona Living Well Program at the Arizona Department of Health Services to deliver the Healthy Living: Self-Management of Chronic Conditions Program throughout the state of Arizona.  Healthy Living is a six week workshop developed by Stanford University to help participants learn to self-manage their on-going health conditions.  The Institute will be working to connect those in the community who are interested in the Healthy Living Workshops with those who are delivering the workshops.  We are working to coordinate the efforts of our Arizona communities to help improve the lives of adults living with chronic diseases.

About the Healthy Living: Self-Management of Chronic Conditions Program
  • The Healthy Living: Self-Management of Chronic Conditions program is a workshop developed by Stanford University to help adults manage their on-going health conditions.
  • The workshop designed to help people who have a chronic disease or condition gain self-confidence in their ability to control their symptoms & learn how their health conditions affect their lives.
  • Small-group workshops are 6 weeks long, meeting once a week for 2.5 hours.
  • The workshop teaches management skills through interaction, goal setting and action planning.
  • Healthy Living workshops are taught by trained leaders who often live with chronic conditions.
